(PHQ-9)

The PHQ-9 is a self-report scale designed to measure the severity of depression symptoms over the past two weeks. It consists of nine items that assess various problems commonly associated with depression. Please read each item and select the number that best reflects how often you have been bothered by that particular problem over the past two weeks. There are no right or wrong answers. Choose the response that most closely applies to your experience.

0 - Not at all
1 - Several days
2 - More than half the days
3 - Nearly every day

6. Feeling bad about yourself - or that you are a failure or have let yourself or your family down?

7. Trouble concentrating on things, such as reading the newspaper or watching television?

8. Moving or speaking so slowly that other people could have noticed? Or so fidgety or restless that you have been moving a lot more than usual?

9. Thoughts that you would be better off dead, or thoughts of hurting yourself in some way?
